CVATD can provide you with a Certificate of Completion for attending our programs at your request.

Simply fill out this form and the CVATD team will verify your participation and send you a PDF certificate.

This gives you an easy way to keep track of CPTD/APTD recertification points while also providing you with documentation of attendance should you need it.
